Documents to prepare
- The documents are requested through the notice you get from Maryland Health Connection (MHC). For aged, blind or disabled (ABD) coverage and long-term care (LTC), asset (resource) and medical requirements may apply. The linked long-term care form 9709 (July 2017 edition) asks for your financial and asset history and says to apply with the copies you have instead of waiting until you have every document. Whether this form is still accepted, and the exact document list for aged, blind or disabled coverage, are still being checked
